Comprehensive Plan (offers coverage for diagnostic, basic and major services)
High Option Plan (offers coverage for the same services as the Comprehensive Plan with richer benefits plus coverage for orthodontic care*)

Check in without an ID card

Just provide your name, birth date and ID or Social Security number.
Family members covered under your plan will need your information.
Prefer to show a paper or electronic dental ID card? Sign into Delta Dental’s online services to print or email your card.
